1. Acceptance of Terms

By accessing or receiving services from Eternal Health (“Clinic”, “we”, “us”, or “our”), you (“you”, “your”, “patient”) agree to be bound by these Terms & Conditions in full. If you do not agree with any part of these terms, you must not use or access our services.

2. Services

We provide medical wellness services including, but not limited to:

  • Testosterone Replacement Therapy (TRT)
  • Medical Weight Loss
  • Vitamins and Supplements
  • Botox Treatments
  • Injection Bar Menu
  • Skin Care

The specific services available to you may vary over time, and we reserve the right to add, modify, or discontinue any service at our discretion.

3. Appointments & Payment

  • Payment is generally due at the time of service unless other arrangements are agreed upon in writing.
  • Eternal Health does not guarantee insurance coverage or reimbursement. You are responsible for all charges if your insurance does not cover the service.
  • Missed or late appointments may be subject to a cancellation or no-show fee in accordance with our office policies.
  • Payments for services rendered and products dispensed are typically non-refundable, except as required by applicable law.

4. Medical Evaluation & Informed Consent

  • Before starting treatment, our provider will review your medical history, current medications, and may order laboratory testing as needed.
  • You will be informed of the potential benefits, risks, side effects, and alternatives, and you agree to provide accurate and complete health information.
  • You understand that some therapies may constitute “off-label” use of medications and that individual results will vary. No guarantee of outcome is made or implied.
  • You agree to follow the provider’s recommendations for follow-up visits, monitoring, and lifestyle changes to optimize safety and effectiveness.

5. Risks & Limitations

All medical treatments carry inherent risks. Potential risks, side effects, or complications will be discussed with you prior to treatment where reasonably possible. By proceeding, you acknowledge:

  • You may experience side effects or complications even when care is appropriate.
  • You must immediately report new or worsening symptoms, adverse reactions, or hospitalizations to the Clinic.
  • Our services are focused on wellness and quality of life and may not be considered medically necessary in all circumstances.

6. Insurance & Financial Responsibility

  • We may provide itemized receipts that you can submit to insurance; however, we do not guarantee reimbursement.
  • You remain financially responsible for all charges associated with your care at Eternal Health, including services that are denied, not covered, or applied to deductibles by your insurer.

7. Privacy & Confidentiality

Eternal Health maintains the privacy and security of your health information in accordance with applicable federal, state, and local laws, including HIPAA. Your information may be shared with:

  • Laboratories, pharmacies, or other healthcare providers involved in your care;
  • Third parties as necessary for billing, collections, or healthcare operations; and
  • Regulatory authorities or as otherwise required by law.

8. Website, Content & Intellectual Property

All content on our website, printed materials, and digital content created by Eternal Health is owned by or licensed to us and is protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from our content without prior written permission.

Our online content is provided for general information only and is not a substitute for personalized medical advice or diagnosis.

9. Limitation of Liability

To the fullest extent permitted by law, Eternal Health and its providers, staff, owners, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services or website.

Where liability cannot be disclaimed, our total liability to you for any claim shall not exceed the amount actually paid by you to Eternal Health for the specific service giving rise to the claim.

10. Indemnification

You agree to indemnify, defend, and hold harmless Eternal Health, its providers, staff, and affiliates from and against any claims, damages, losses, liabilities, and expenses (including reasonable attorneys’ fees) arising from:

  • Your breach of these Terms & Conditions;
  • Your failure to provide accurate health information; or
  • Your misuse of our services or website.

11. Changes to Terms & Services

We may update these Terms & Conditions from time to time. When we do, the “Last Updated” date at the top of this page will be revised. Your continued use of our services after any changes signifies your acceptance of the updated terms.

12. Governing Law & Jurisdiction

These Terms & Conditions are governed by the laws of the State of Nevada. Any dispute arising out of or related to these terms or our services shall be brought exclusively in the appropriate state or federal courts located in or serving Carson City, Nevada.

13. Severability

If any provision of these Terms & Conditions is held to be invalid or unenforceable by a court of competent jurisdiction, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.

14. Entire Agreement

These Terms & Conditions, together with any consent forms, office policies, and other written agreements between you and Eternal Health, constitute the entire agreement regarding your use of our services and supersede any prior or contemporaneous communications.
